New Post: Output excel if POST variable

Hey, I would like to get data from FORM and use it to my excel file.

First of all, I have included PHPExcel.php and IOFactory.php files. It seems its okey.

I just want to call excel output if POST status. But after click button who send data no effect. Where is my mistake?
